When referring a patient our goal is to work closely with you to ensure continuity of care by offering advanced diagnostic testing and treatment regimes. Our goal is to create an uncomplicated experience for you and your patients.
Lisa Tomlinson, our Referral Coordinator, is available to help with the following:
- expedite making an appointment for your client
- gather appropriate medical history
- provide daily updates to you regarding hospitalized pets
- setup outpatient ultrasound appointments
Prior to your client’s visit, please email records to email@example.com or fax to (508)638-6299, the following information:
- Referral Form for NEAMC Emergency & Critical Care, Surgery and Internal Medicine
- Abdominal US/Echocardiogram Referral Form
- Recent patient history and any laboratory tests
- Radiographs (send hard copy or disc with patient or email to firstname.lastname@example.org)
The quickest and easiest way to refer a patient to one of our specialists is to contact us directly by phone at 508-584-1600 and ask to speak with our Referral Coordinator. Our specialists are available for a phone consultation with veterinarians prior to referral, if desired.
Our policy is to create a relationship of trust with our referring doctors. We want to work directly with your practice to coordinate the best possible care for your patients. We respect the relationship between you and your client, and it is our goal to ensure that professional communication continues between our office and yours, while your patient is temporarily in our care.

Symbiosis is defined as a relationship with mutual benefits to two or more individuals. We hope that is an accurate description of the relationship between your practice and ours. We have two primary goals. The first is to provide you and your clients a 24/7 emergency and critical care resource where skilled and compassionate care is available for your patients when you are not on duty. Our second goal is to provide you with a nearby resource for specialist care.
